Are you new to the district or have a kindergarten student to enroll? Summer registration for 2013-14 will take place during July 8-25 at Boulder Creek HS and Park Meadows. Office hours will be Monday – Thursday from 7:30 a.m. – 5 p.m. To find out which school your child will attend, refer to the boundary map section on the district website,

www.dvusd.org. DVUSD is currently accepting open enrollment applications for 2013-14. Applications may be obtained from the district office or online at the district’s website. For more information call 623-445-

5000. Parents will need to furnish the following information for all students:

• A birth certificate or a certified copy

• An immunization record

• Proof of residency (utility bill, rent receipt, lease agreement, escrow doc., etc.)

• Legal guardian or custody papers, if applicable

• Official withdrawal form from the previously attended school (1st-12th)

School attendance is not only a good habit, state law requires it. Arizona State Law (15-802.A, 15-803.E) requires every person who has custody of a child between the ages of six and sixteen years shall make sure the child attends school for the full time school is in session unless unable to attend due to illness or another legitimate reason.
